 This document allows for you to create an advanced care directive – a way to let people know about your healthcare and medical treatments, if you find yourself in a position where you are unable yourself, to make these decisions. Having an advanced care directive aids your loved ones and medical professionals if they one day need to make decisions for you.  

 Before you get started, you may want to consider these issues:

·         Who would you like to make medical and heath decisions for you if you are unable to? 

·         What beliefs and goals you share and your preferred medical/social outcomes? 

·         Any religious or cultural matters related to the medical or health field. 

·         The treatments or care you would like, or not like in the face of life-threatening illness or injury.
